Purpose of the Role
We are seeking a qualified Social Worker to support young adults transitioning out of care. The role focuses on life planning, family reunification, community integration, and creating pathways to employment, while strengthening support for vulnerable families.
Key Responsibilities
· Develop and implement life plans for young adults (18+)
· Provide psychosocial support, mentorship, and independent living skills training
· Facilitate life skills programmes and workshops
· Build partnerships with NGOs, employers, and community stakeholders
· Prepare and support families for reunification, including ongoing follow-up
· Facilitate supervised family contact where required
· Support access to employment opportunities and provide job coaching
· Engage with communities and link families to resources
· Maintain accurate case records and participate in case management processes
Minimum Requirements
· Bachelor’s Degree in Social Work
· Registered with SACSSP
· 3–5 years’ experience in reunification & community work
· Valid driver’s licence and willingness to travel to underprivilleged communities
· Multilingual – English/Xhosa/Afrikaans
· Experience in working with neurodivergent children/ young adults (advantageous)
