Home-Based Services Program Specialist (Eldercare)

Position Summary
The Human Resources Center (HRC) is seeking a compassionate and organized Program Specialist to support individuals, older adults, and families receiving Home-Based Services & Eldercare services. This position is responsible for coordinating services, supporting consumers and caregivers, maintaining regulatory compliance, and ensuring high-quality, person-centered care that promotes independence and community living.
Key Responsibilities
- Coordinate and monitor home-based services for consumers, including older adults participating in Eldercare programs.
- Conduct assessments, maintain documentation, and support individualized service planning.
- Collaborate with consumers, families, caregivers, referral sources, and community partners to ensure effective service delivery.
- Assist with staff recruitment, scheduling, training, supervision, and ongoing support.
- Monitor program compliance with agency policies, licensing standards, and funding requirements.
- Review service documentation, billing information, and program records for accuracy and completeness.
- Participate in audits, meetings, trainings, and quality improvement activities.
Qualifications
- Bachelor’s degree in Human Services or a related field preferred; equivalent education and experience considered.
- Experience working with older adults, individuals with disabilities, human services programs, case management, healthcare, or community-based services preferred.
- Strong organizational, communication, and problem-solving skills.
- Valid driver’s license and ability to travel throughout the service area.
- Ability to obtain and maintain all required clearances and certifications.
