Responsibilities
- Conduct comprehensive patient assessments to determine needs, symptoms, and care priorities with attention to physiology and disease progression.
- Develop and implement individualized care plans aligned with patient goals, ensuring adherence to HIPAA regulations and medical records standards.
- Manage symptom control and palliative interventions, providing emotional support and education to patients and families.
- Coordinate discharge planning, post-acute care arrangements, and outpatient services to ensure continuity of care.
- Monitor vital signs, medication administration, and diagnostic evaluations while maintaining accurate documentation in medical records.
- Collaborate with interdisciplinary teams including physicians, social workers, chaplains, and case managers to deliver holistic hospice care.
- Educate patients and caregivers on disease management, medication use, and advance directives to empower informed decision-making.
Experience
- Proven nursing experience with a strong background in critical care settings such as ICU, emergency medicine, or trauma centers (Level I or Level II Trauma Center experience preferred).
- Hands-on experience in hospital medicine, medical-surgical units, or post-acute outpatient clinics demonstrating patient service excellence.
- Familiarity with infectious disease care protocols, behavioral health considerations, geriatrics, pediatrics (PICU), or OR experience is advantageous.
- Knowledge of hospital experience including intake processes, utilization management, diagnostic evaluation, case management and inpatient/outpatient workflows.
- Strong understanding of physiology principles relevant to acute care environments combined with excellent patient assessment skills.
- Experience working within managed care frameworks while adhering to HIPAA standards for medical records confidentiality.
- Ability to perform comprehensive patient evaluations including vital signs monitoring and clinical documentation in fast-paced settings such as emergency rooms or inpatient units. Pay: $83,471.73 – $95,525.09 per year
