Position Overview
We are seeking a compassionate and community-oriented Registered Dietitian Nutritionist (RDN) to support participants enrolled in the Social Care Network (SCN) program under the 1115 Medicaid Waiver.
This role focuses on addressing health-related social needs (HRSN), including food insecurity and nutrition access for Medicaid members. The RDN will work closely with interdisciplinary teams and community partners to assess client needs, provide nutrition support, and facilitate access to appropriate food resources.

Key Responsibilities
- Conduct nutrition and health-related social needs (HRSN) assessments for Medicaid members
- Evaluate clients’ food access, dietary needs, and eligibility for nutrition services
- Develop and implement individualized nutrition care plans
- Provide medical nutrition therapy for:
- Chronic disease management (diabetes, hypertension, obesity)
- Prenatal and maternal nutrition
- Behavioral health and substance use support
- Coordinate food boxes, groceries, or nutrition resources through community partners
- Deliver culturally appropriate nutrition counseling and education
- Collaborate with care coordinators, social workers, and community-based organizations
- Monitor client progress and adjust care plans as needed
- Maintain accurate documentation in accordance with 1115 Waiver requirements
- Participate in virtual case discussions and interdisciplinary team meetings
Education Requirements
- Bachelor’s or Master’s degree in Nutrition, Dietetics, or a related field from an ACEND-accredited program
Qualifications (Required)
- Active Registered Dietitian Nutritionist (RDN) credential
- Active New York State Certified Dietitian-Nutritionist (CDN) — required
- Active NPI number
- Bilingual Spanish
- Experience working with Medicaid populations, community health, or food insecurity programs preferred
- Strong communication and cultural competency skills
- Ability to work independently in a remote environment
- Experience with electronic health record (EHR) systems preferred
Compensation & Schedule
- Full-Time (W-2) position
- Hourly Rate: $38.00 – $48.00 per hour (based on experience)
- Consistent full-time schedule (40 hours per week)
- Flexible scheduling within standard business hours
