What You’ll Be DoingDeliver Exceptional Rehabilitation Services
- Perform comprehensive patient evaluations and develop individualized care plans
- Deliver evidence-based therapeutic interventions
- Provide caregiver education and equipment recommendations
- Collaborate with nurses, physicians, and interdisciplinary teams
- Accurately document visits and monitor patient progress
Required Qualifications
- Active Minnesota Physical Therapist license
- Willingness to travel throughout Southwest Minnesota
- Strong communication and clinical reasoning skills
- Ability to work independently and manage your schedule effectively
Preferred Qualifications
- Home health experience
- Experience with medically complex patients
- Passion for relationship-based, patient-centered care
